Injury Tracking Process & Report
All injuries MUST be reported to Mendham Little League's Director of Safety, Ed Powers. Please print and complete the attached form.
Please call the Director of Safety at 973-543-1559 immediately after securing the necessary medical treatment for the injured. You must also fax a copy of the injury tracking report to 212-653-2245 within 24 hours of the injury.
